Will my rapid antigen test be covered by insurance?

As of January 15th, 2022, private health plans must cover 8 individual at-home over-the-counter COVID-19 tests per person enrolled in the plan per month. Individual at-home tests are reimbursable up to $12 per test unless the health plan provides direct coverage. In some cases, multiple tests are packaged together (e.g. serial testing). A two-pack of rapid COVID tests would therefore be eligible for reimbursement up to a total cost of $24.

Once your order is complete, you can apply for reimbursement directly from your health insurance provider. Your health insurance provider will let you know what information needs to be submitted with your reimbursement request and it may include the following:

  • Your patient information
  • Manufacturer of the test
  • Where the test was purchased
  • Date of purchase
  • Cost of the test
  • CPT code 87426 – Infectious agent antigen detection by immunoassay technique, (eg, enzyme immunoassay [EIA], enzyme-linked immunosorbent assay [ELISA], immunochemiluminometric assay [IMCA]) qualitative or semiquantitative, multiple-step method; severe acute respiratory syndrome coronavirus (eg, SARS-CoV, SARSCoV-2 [COVID-19]).
