Tips

    • Purchasing Everlywell 360 on Everlywell.com

    • Choose the appropriate panel based on your sex assigned at birth.
    • Check out with information that matches your photo ID (first name, last name, date of birth, etc.).
    • Use a valid email address and phone number. This is how we will send you updates on your progress, let you know your results are ready, and communicate next steps.
    • You can take this test even if you are pregnant, nursing, or on hormone therapy or other medications. However, keep in mind that this may generate results that are outside the reference ranges, so you should consult with a healthcare provider about any concerns before taking this test.
    • Keep in mind, you must be 18 years or older to purchase this test. Membership is not currently offered in AZ, AK, HI, NJ, NY, or RI.

    • Remember that you will need to fast for 9-12 hours prior to your labs, so you might not be able to make a same day appointment if you have already eaten. If you do not fast, you still can get your blood drawn, but results may be outside of reference ranges.
    • Use the same information you placed your order with. This should match the photo ID you will need to bring to your lab appointment.

    • Check in ahead of time for your appointment. Quest will remind you about your appointment and provide a link to do this.
    • Familiarize yourself with your MyEverly Dashboard and fill out your Health Profile.
    • Be prepared to present your photo ID.
    • If Quest is unable to locate your appointment or lab order, make sure they have the right name and date of birth information. If it’s possible you entered an alternate (like a maiden name or nickname), please provide that as well so they can better search for you.
    • If they continue to experience issues, don’t leave! Call us first at 1-833-998-5465. We should be able to help get your lab order faxed to the Quest Diagnostics location.
    • When you’re at Quest Diagnostics for your labs

    • Be prepared to present your photo ID.

    • Do NOT pay Quest directly for any labs or services. There are no additional charges once you’ve completed your purchase on Everlywell.com.
    • Do NOT create a MyQuest account.
    • You do NOT need to pay attention to any emails and/or text messages you receive from Quest.
    • You do NOT need your insurance card.
    • Do NOT leave if they can’t find your appointment or lab order. Instead, provide them with any iterations of your name or date of birth you may have entered, and then call us for help at 1-833-998-5465.

      How were the tests for Everlywell 360 selected?

    • Our in-house clinical experts curated this comprehensive set of lab tests to offer an analysis of your whole-body health and help monitor early signs of disease.

      Are Everlywell 360’s tests accurate?

    • All tests are processed in CLIA-certified and CAP-accredited labs through Quest Diagnostics. Your blood and urine samples are collected by licensed medical technicians.

      Does Everlywell still offer at-home lab tests?

    • Yes, we still offer several convenient at-home lab tests from our Food Sensitivity Test to sexual health tests, hormone tests, and more.

Ordering and Scheduling

    • Is there an age requirement to join Everlywell 360?

    • Yes, you must be over 18 years of age to join Everlywell 360.

      Which states can I purchase an Everlywell 360 membership?

    • Membership is not currently offered in AZ, AK, HI, NJ, NY or RI.

      What happens after I order my test?

    • After purchase, you will be logged in to your MyEverly dashboard to see your lab order status. Once the lab order is processed by the independent physician network, you will receive an email with instructions on how to schedule your appointment at any Quest Diagnostics® near you. You can also walk in to a Quest Diagnostics® for sample collection at your convenience once the lab order is approved.

      How do I schedule my appointment?

    • You can schedule an appointment at your preferred Quest Patient Service Center by using the link in your MyEverly dashboard, or by calling 833-998-5465.
    • Appointments are strongly encouraged and will receive priority. Walk-ins may be accommodated during the next available appointment opening, but cannot be guaranteed service. Scheduling an appointment will also give you the option of getting helpful pre-appointment email reminders and tips from Quest. If you prefer to walk in, it helps to know that Quest Patient Service Centers are usually busiest in the early morning. We recommend calling the location in advance to confirm they support walk-ins.
    • Please bring a government issued photo ID (drivers license or equivalent) when checking in for your appointment. It must match the information provided when you purchased your test.
    • Steps for booking online:
      1. Login to your Everlywell account and click the “Schedule now” button on your MyEverly dashboard.
      2. Fill out your personal information
      3. Enter your zip code or address to find a location near you.
      4. Pick a date and select a time slot at your preferred location.
    • How do I reschedule an appointment or cancel an appointment?

    • You can view, change, or cancel your appointment by visiting the Quest website and clicking: view, change or cancel an existing appointment.

      How long do I have to schedule my appointment after I order? Does my order ever expire?

    • All in-person appointments must be completed within 12 months of purchase.

      Is there anything I should bring with me to my appointment?

    • Please bring a government issued photo ID (drivers license or equivalent) Your ID must match the information you provided when purchasing your test.

      Are there any costs associated with the appointment/test?

    • All fees are included in the purchase price of your test. You do not need to pay anything additional for the appointment or provide insurance information.

      If I have questions about my appointment, who do I contact?

    • Questions about your appointment can be answered by Quest, and questions about your purchase, results, or treatment can be answered by Everlywell Customer Care or Everlywell Concierge line at 833-998-5465.

      How long will the appointment take? 

    • Wait times can vary by time of day and location, so it is recommended to schedule an appointment in advance. However, most customers are seen and served in under 20 minutes. If Quest needs to clarify or update your test order by calling Everlywell Customer Care, it may take longer.

Preparing for Test

    • I recently had my annual lab work done. Should I wait to join Everlywell 360?

    • Over time, your health status might change. Everlywell 360 tests for 83 biomarkers, which is 3x more than your average annual physical.

      Where do I go for my lab visit?

    • You can conveniently schedule your lab visits at one of 2,000+ Quest Diagnostics locations.

      Do I need to fast before testing?

    • Yes, it is required that you fast for 9-12 hours prior to taking your test.

      How do I prepare for this test?

    • Fasting Requirement: Do not eat or drink anything (except water) for at least 9-12 hours before your lab visit. This helps ensure accurate test results.
    • Hydration: Drink plenty of water before your appointment to make it easier to provide a urine sample.
    • Required Samples: Be prepared to provide both a blood sample and a urine sample during your visit.
    • Photo ID: Make sure to bring your photo identification with you to match your appointment and order information.
    • Timing: Morning appointments are recommended for the most accurate iron and cortisol test results. 
    • Medications and Supplements: Some tests may be affected by supplements containing biotin. We recommend testing 8 or more hours following the last biotin supplementation. While your results would be accurate, please keep in mind, medications containing hormones or corticosteroids will have an impact on your hormone levels. Speak to your healthcare provider before making changes to any medication usage. Please do not discontinue use of any medications that you are taking without first speaking to your healthcare provider.
    • Important Note for Women: For premenopausal women, we recommend completing your lab panel between days 19-21 of your cycle, before your menstrual period.  Testing during your menstrual period is not recommended as this can affect the accuracy of your urine results. 

      Important Note for Everyone: You DO NOT need your insurance card and will not be required to pay for this lab work at the testing center. Disregard emails and/or texts from Quest that indicate differently.

      How are the samples taken?

    • Your samples will be taken at a Quest Diagnostics location. At Quest Diagnostics, a healthcare provider will take your sample by drawing blood from a vein in your arm, and you will self-collect a urine sample. 

      Does getting my blood drawn have any side effects I should be aware of? 

    • Having your blood drawn has minimal side effects. You may experience bleeding, bruising, or soreness at the puncture site, lightheadedness, or rash or skin irritation from the bandage. These side effects will go away over time.

Results and Test Information

    • How long before I get my results back?

      Most customers get their lab results back within 9 days after lab work is completed, and your WellScore is delivered with your results. Your customized Care Plan, which includes a clinical summary and guidance on next steps for your wellness journey, should be ready within 24 hours of your results being made available.

      Can Everlywell diagnose me based on my test results?

      No, diagnosis and treatment are not currently available through Everlywell 360. However, your results will come with a comprehensive clinical analysis and will be easy to share with your primary care provider.

      Will being on hormonal birth control affect my test results?

      Your results will accurately reflect your hormone levels on your current medications. Oral contraceptives and hormone replacement therapy impact your hormone levels (estradiol, progesterone, testosterone, FSH, LH), so your results will be different than what you would see if you were not taking these medications.

      How will I receive my results?

      You will receive an email to view your results in your MyEverly account.

      Why might my Everlywell 360 lab results potentially differ from my at-home lab results?

    • A laboratory threshold or reference range is used to determine expected lab values for the average person in the population getting tested (such as adults, males, females, etc.).
    • Laboratories establish their own reference ranges based on many factors, including:
      • Internal controls
      • Testing methodology
      • Assay validation
      • General population data
    • Although reference ranges from different laboratories are likely similar, there can be differences in what is considered “normal” when testing is performed. 
    • Differences in the units of measurement, sample type, and collection timing can also influence the reference range. For example, the same biomarker measured in serum versus saliva would have different reference values, as would the same biomarker and sample type, but with certain different age groups.
    • When comparing test results, it's important to look at where your levels are within the reference range provided by that individual lab–for example, whether levels were low, high, or normal within that given reference range, as opposed to looking at the specific numeric outputs (which may also have different units). It is not accurate to examine your results under the context of other reference ranges, whether from other labs or online, because the numerical outputs are specific to the provided reference ranges that accompany them.
    • In general, it's often how results trend over time or where your levels are within the range itself that determines next steps or if a follow-up is needed with their healthcare provider. You can find more information related to reference range variability in our blog.

Reimbursement / Refunds / Insurance

    • Can I use insurance to pay for my 360 membership?

    • At this time, you cannot use insurance to pay for Everlywell 360. However, our membership only costs $399 annually, which is far less than you might pay in out-of-pocket costs through a doctor without insurance. We estimate the out of pocket costs of these tests to be about $10,000.

      Does Everlywell 360 accept HSA/FSA payments?

    • Your Everlywell purchase can be submitted for reimbursement through HSA/FSA benefits. As always, be sure to check with your specific HSA/FSA benefits coordinator for clarification on the specific requirements for reimbursement including usage, protocol, and qualifications.

      How do I cancel my Everlywell 360 membership?

    • You can cancel your membership at any time by contacting our Everlywell 360 Concierge team at everlywell360@everlywell.com.

      How does payment for Everlywell 360 work?

    • Everlywell 360 is an annual membership program, so you’ll be charged annually on the same day you signed up and be prompted to schedule your lab tests again through Quest Diagnostics.

Consult Offering

    • I did not purchase the consult offering when I bought my 360 Everlywell membership. Can I add it on now?

    • While you can’t purchase it after your results come back just yet, we are actively working on making this available soon! We’ll share next steps once we have this available.
    • I purchased Everlywell 360 + Consult, but I’m not able to book my visit yet. When will I be able to book my visit?

    • After you receive your lab results and initial clinician-reviewed Care Plan, you’ll be able to book your visit on your MyEverly dashboard right when you login.
    • What do I get in an Everlywell 360 consultation?

    • With an Everlywell 360 consultation, you'll meet 1:1 with a licensed clinician to review your lab results, which include insights across 83 key biomarkers. During your consultation, your clinician will:
      • Explain what your results mean in the context of your overall health
      • Answer any questions you have about your Care Plan or specific results
      • Provide personalized guidance based on your results, symptoms, medical history, and health goals
      • Recommend next steps, which may include lifestyle changes, follow-up testing, medication treatment if appropriate, or referrals for additional care
    • This is a dedicated time to better understand your body and make informed decisions about your health, with support from a trusted professional.
    • How does an Everlywell 360 consultation differ from the results I get back after my labs?

    • For many people, the care plan included with your lab results will provide the insights and next steps needed—especially if your results are normal or only slightly outside of range. However, an Everlywell 360 consultation can offer additional value in certain situations:
      • If you have multiple or significantly abnormal results and want a more in-depth explanation
      • If you have a complex medical history and would like a clinician to interpret your lab results in the context of your personal health
      • If you’re experiencing ongoing or chronic symptoms and want to explore how your lab results may relate to what you’re feeling
      • If you’re exploring prescription medication based on your results, consultations offer an opportunity to discuss potential treatment options
      • If you’re curious about a specific diagnosis or seeking deeper understanding that goes beyond the recommendations provided in your Care Plan 
      • If you value the added context, guidance, and reassurance that a licensed clinician will be able to offer in a live 1-on-1 consultation

        What sort of next steps and action plan will I receive from my Everlywell 360 consultation?

      • Customized Consult Summary: Your action plan may include lifestyle recommendations, further evaluation steps, specialist referrals, and guidance on prioritizing goals based on your unique lab profile.
      • Medication Recommendations (if medically appropriate): If your results indicate a condition that may benefit from treatment, your clinician may recommend or prescribe medications, if appropriate.
      • Timeline and Follow-Up Guidance: Your clinician will outline how to monitor progress, including:
        • When to re-test your labs
        • What improvements to track (energy, mood, weight, symptoms, etc.)
        • When to consider a follow-up consultation for re-evaluation or continued support
    • What if I need to re-test various biomarkers/require a prescription to treat a condition that is revealed in my Everlywell 360 labs?

    • If your Everlywell 360 lab results reveal something that needs follow-up—like an abnormal value or a condition that could benefit from treatment—your consultation clinician will guide you through your options.
      • Your clinician may recommend re-testing specific biomarkers to:
        • Monitor progress toward your health goals
        • Confirm a diagnosis
        • Track your response to treatment
    • If medically appropriate, your clinician can prescribe treatment directly during your consultation. Common examples include:
      • Hypothyroidism: Levothyroxine
      • High cholesterol (hyperlipidemia): Statins
      • Iron-deficiency or anemia: Iron supplements, with follow-up testing
      • UTIs: Antibiotics
      • Pre-diabetes/diabetes: Metformin
      • Vitamin/Mineral deficiencies: Supplements like Vitamin D, B12, Magnesium, or Zinc
    • Is it guaranteed that I’ll receive testing recommendations or a prescription during the consultation with my clinician?

    • No, receiving testing recommendations or a prescription is not guaranteed. Your clinician will evaluate your health concerns during the consultation and determine the most appropriate course of care. Prescriptions are issued only when clinically appropriate and at the sole discretion of the clinician.

Refunds

    • I purchased the Everlywell 360 + Consult membership but am no longer interested. Can I get a refund?

    • You have up to 60 days after you order to cancel your purchase. We do charge a non-refundable cancellation fee of $20, which includes administrative and service fees. No purchases can be canceled after sample collection at one of the Quest Diagnostics Patient Service Centers has occurred. 
      • If you’ve already gotten your lab results back, and your consultation has not yet occurred, you are eligible for a partial refund of $100, minus the cancellation fee of $20. Contact our customer service team at 888-813-0230 or email us at contact@everlywell.com, Monday - Friday, 8 a.m. - 4:30 p.m. CST. 
      • If your visit has already occurred, you are no longer eligible for a refund.

Cancellations and Rescheduling

    • I’d like to cancel or reschedule my scheduled consultation. How can I do that?

    • You can cancel or reschedule your consultation by logging into your MyEverly account. Please note that cancellations within 24 hours of your scheduled consultation may incur a $25 cancellation fee. You can then re-book your consultation when you’re ready.
    • I missed my scheduled consultation! How can I book a new one?

    • We understand that things come up. If you are unable to make your consultation, we ask that you give us 24-hours notice. If you do not cancel your consultation and are unable to make your visit, you may incur a $25 charge. You can then re-book your consultation in your MyEverly account or by contacting our customer service team when you’re ready to reschedule.

Critical Results

    • I received a call from a nurse about my results - can I still book my consultation?

    • Please follow the guidance you received from the nurse first before booking a consultation with a clinician. For instance, your nurse may have advised you to seek in-person care. After following the guidance provided by the nurse, you may then also wish to book a visit with one of our clinicians.

Clinicians

    • What are my clinician’s credentials?

    • All of our clinicians are licensed healthcare professionals with extensive telehealth experience. Our care team includes Medical Doctors (MDs), Doctors of Osteopathic Medicine (DOs), Nurse Practitioners (NPs), and Genetic Counselors. Our practice has 50 state licensures. Each clinician has been carefully vetted for clinical expertise and telehealth proficiency, ensuring you receive high-quality personalized care no matter where you’re located.

Follow Up Care

    • Are additional labs or prescriptions that my clinician orders included in my membership fee?

    • No, additional labs or prescriptions are not included in the cost of your membership. We will enable repeat testing of key biomarkers and repeat consults in the very near future. If you are interested in this, we can email you to let you know when this feature will be available.
