How to collect my STD Test - 5 Panel urine sample

Note: The Chlamydia & Gonorrhea and Trichomoniasis Tests require a different type of sample collection. You can find those collection videos here.

Important Notes Before You Begin

  • DO NOT perform your STD Test – 5 Panel collection while taking antibiotics or within 7 days after finishing antibiotics.
  • DO NOT pour the preservative out of the tube.
  • Wait at least 24 hours after intercourse before collecting your sample.
  • Register your kit online before collecting.

How to Collect Your Urine Sample

  1. Open the urine sample packet.
  2. Place the kit ID sticker on the tube (do not cover the “Fill area” window).
  3. Expand the green collection cup and urinate into the cup.
  4. Use the pipette to transfer urine from the cup to the tube. Fill between the lines to provide enough sample to test. DO NOT overfill.
  5. Tightly seal the cap and gently invert the tube several times to mix the urine with the preservative.
  6. Place the urine sample inside the biohazard bag with the absorbent sheet.
  7. If your test requires a blood sample in addition to a urine sample, follow the instructions inside the kit or watch the dried blood spot collection video here.

How to Return Your Sample

  1. Locate the return envelope and shipping label at the bottom of the box.
  2. Place the biohazard bag back into the box for extra protection.
  3. Put the box inside the return envelope and attach the shipping label.
  4. Send back your sample right away using the shipping carrier listed on your label.
